HRS is looking to add a Student Nurse intern to our team. This Student Nurse intern role will be onsite at Main Line Health, while being employed by a partner of MLH, Health Recovery Solutions, a Villanova alumni founded remote patient monitoring company. The company’s technology is in use at MLH and this role would have hands-on work with not only providing patient care, but learning the ins and outs of working at a private equity backed digital health company.
The Student Nurse Intern has the responsibility and accountability for providing coordinated, comprehensive nursing care to patients in the home setting, in collaboration with the HomeCare & Hospice RN Case Manager (Staff Nurse, HCH). They demonstrate critical thinking, analysis, and clinical judgment in all aspects of care delivery for patients and caregivers facing acute, chronic, and life limiting illness. This includes use of the nursing process to address the physical, psychosocial, and spiritual needs of patients and families to improve function, independence, and quality of life. The Student Nurse Intern integrates the practice of nursing through the implementation of the nursing process, in assessing and rendering physiological and psychological care for patients.
In collaboration with the HomeCare & Hospice Advanced Illness Management (AIM) High Risk Coordinator, the Student Nurse Intern assists with the management of specified clients including, but not limited to, those on the Remote Patient Monitor (RPM), Telehealth, and other Advanced Illness Management programs through the provision of telephonic and RPM clinical assessment and health education. They provide direct contact with clients who have been identified as high risk for illness exacerbation and rehospitalization. They utilize health coaching, motivational interview, and self-efficacy strategies to set patient centered goals. The Student Nurse Intern assesses telehealth and RPM data and provides a response related to the data in collaboration with the AIM High Risk Care Coordinator.
Assessment
- Conducts comprehensive and timely nursing assessment/ reassessment and collects data in a systematic manner using critical thinking, analysis, and judgment for assigned patients.
- Involves the patient, family, other healthcare providers and the environment in holistic data collection. Uses evidence-based assessment and instruments in collecting data and gathering assessment findings.
- Analyzes assessment data to identify actual and/or potential physiologic, holistic, cultural, and psychosocial problems and needs to contribute towards development of the interdisciplinary plan of care in collaboration with the HomeCare & Hospice RN Case Manager.
- Utilizes technology tools including, but not limited to, remote patient monitoring to enhance assessment and data collection findings for assigned patients. Performs electronic and telephonic assessment of identified client’s physical, psychological, functional, environmental, social, and emotional health status. Assesses symptom burden, medication adherence, disease progression, changes in vital signs and/or weight, and other patient/ family needs.
- Retrieves and analyzes client specific data available from the RPM system and/or client electronic medical record in collaboration with the AIM High Risk Care Coordinator.
- Reviews telemonitoring system reports to ensure clients are adherent and trends are reported in a timely manner.
Planning/Patient/Family Education
- Consults with patient/family and appropriate health care professionals to meet identified needs.
- Provides education to patient/family; evaluates teaching, coordinates learning with health care team and documents completed teaching and response.
- Provides education to patient/ family regarding the use and benefits of remote patient monitoring. Teaches patient how to utilize equipment and tools to monitor health condition.
Implementation
- Maintains knowledge required to provide safe, therapeutic, and effective patient care.
- Implements the identified plan of care in a safe, timely, and culturally competent manner. Utilizes evidence-based interventions and treatments specific to the diagnoses or problems. Employs strategies to promote environmental safety, restoration of health, and prevention of illness, injury, and disease. Identifies and collaborates with HomeCare & Hospice RN Case Manager and AIM High Risk Coordinator for changes in patient condition.
- Coordinates care delivery and implementation of the plan of care in collaboration with the HomeCare & Hospice RN Case Manager and other interdisciplinary professionals. Ensures smooth transitions across all care settings. Identifies the need for community resources and other therapeutic disciplines.
- Evaluates progress towards attainment of goals. Includes patient, family, and others in the care in the evaluative process.
- Provides emotional support to patients/families.

Education
- Minimum Required: Successful progression to Junior or Senior year within accredited BSN program
BLS/ CPR Certification Requirements
- BLS Certification by American Heart Association required.
Licensures and Certifications

- Current CPR certification required.
