Position Overview
Join our award-winning 4-star clinical team as a Home Health RN Case Manager and make a meaningful impact on patients’ lives while serving your community. In this role, you’ll provide exceptional nursing care in the comfort of patients’ homes, focusing on health optimization and hospitalization prevention. Our innovative use of Andy AI technology assists clinicians with OASIS completion and submission within 24 hours, reducing administrative burden and enhancing clinical efficiency.

Key Responsibilities
• Clinical Care Management: Conduct comprehensive patient assessments, develop individualized care plans, and coordinate multidisciplinary healthcare services • Patient Education: Provide expert guidance on disease management, medication compliance, and health maintenance strategies • Documentation Excellence: Utilize WellSky Kinnser EMR and Andy AI technology for accurate, timely clinical documentation and OASIS assessments • Care Coordination: Collaborate effectively with physicians, specialists, and healthcare team members to ensure seamless patient care • Quality Assurance: Monitor patient progress, adjust care plans as needed, and ensure adherence to clinical standards and regulatory requirements • Family Support: Educate and support family members and caregivers in patient care techniques and safety protocols
Required Qualifications
Licensure & Certifications: • Valid Illinois Registered Nurse license • Current CPR certification • Valid driver’s license and auto insurance
Experience & Skills: • Open to new graduates and experienced nurses – comprehensive training provided • Willingness to learn WellSky Kinnser EMR system (training included) • Strong clinical assessment and critical thinking abilities • Outstanding written and verbal communication skills • Patient-centered care philosophy with cultural sensitivity • Strong organizational and time management capabilities • Passion for community service and home-based healthcare
